To make it faster and more convenient for you to get medical advice, you can contact us using our online Patient Triage service, or by calling us.
Click the button to go straight to our online system, and click “I want help for a new or ongoing medical issue.” We’ll get back to you within three working days based on your priority.
Alternatively, you can request an appointment by calling our reception teams at Henwick Halt or Lyppard Grange, Monday to Friday.
Appointments.
How to book an appointment at Severn Valley Medical Practice.
How to request an appointment.
Due to the unprecedented demand for our GP appointments we will no longer be accepting face to face bookings made at our reception desk unless requested by a GP.
All appointment requests must be made by telephone between 8am and 11am, or via our online booking service. Your request will be triaged by a Doctor or an Advanced Nurse Practitioner and dealt with appropriately.
How to see your doctor.
All requests for an appointment on the day will be handled by our clinical team and dealt with accordingly - you will be asked a few questions by our receptionists in order that you may be directed to the correct clinician.
The phone lines are open from 8:00am and we have extra staff manning the phone lines during this busy period. If you wish to discuss any other matters please telephone with queries after 12 noon.
How to see your nurse practitioner.
At both medical centres you may be offered an appointment with our advanced nurse practitioner.
The nurse practitioner will be responsible for taking history, examination, diagnosis and treatment for minor illnesses. She is also able to prescribe. She will of course refer/discuss with a doctor when appropriate.
How to see your nurse/phlebotomist or healthcare assistant.
All appointments for our nursing staff must be booked in advance.
Our Phlebotomists hold regular clinics for blood letting. Our Healthcare Assistants see all new patients for advice re: smoking, weight checks, blood pressure checks, ECG's.
Please let the Practice know if you wish to cancel an appointment. You can find out more on how to do this below.
Patient Triage
You can contact us about a medical, administrative or prescription issue by clicking the button below.
Answer a few short questions and we will get back to you within 2 working days.
Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours - they will not be read on the weekends (Saturdays and Sundays) or on bank holidays, or Monday to Friday from 12:00pm to 8:00am the following day.
Please contact 111 during out of hours or 999 if this is an emergency.
Cancellations
Please always let us know if you don’t wish to take up your appointment. Even if it is at the last minute, we can always fill it. We understand that you may now feel better or something else has cropped up and we are happy to rearrange, but please let us know.
You can cancel by calling 01905 422883 or 01905 616977, or by logging in to your preferred online service (such as the NHS App). Please click the relevant button below to cancel your appointment.
Please note that we do record all incidences of not turning up, and we will be in touch if you miss an appointment. We do reserve the right to ask you to leave the practice list if you won’t help us to help you by keeping to your appointments - as missed appointments cost the NHS money, and make it harder for the rest of our local community to access appointments with us.
Home visits.
Lack of transport is not in itself a reason for a home visit.
If you are too ill to attend then please call the surgery, if possible before 10:30am so that the rounds can be planned. We realise that most people make considerable efforts to get to the surgery and we are grateful for that.
Home visits can take up a considerable time and may result in the doctor being unavailable for emergencies.
Give full details and directions to the receptionist if you do require a home visit and your request will be processed.
If you telephone after 10:30am a visit will only be made the same day if the doctor considers the problem urgent.
Enhanced Access Appointments.
We now offer Enhanced Access appointments with a GP (subject to availability). These appointments are available outside our usual Practice opening hours and are available on weekends.
The appointments will be telephone consultations, meaning you don’t have to travel anywhere to receive care from our GP.
These appointments are pre-bookable by contacting the surgery during our opening times – when calling please ask for an Enhanced Access appointment and our receptionists will be able to help you.
Need help urgently?
Patients with urgent requirements will always be seen on the same day—but if you have an emergency you don’t think is appropriate for us, or we are closed, you should call the following numbers:
NHS 111
Patients who are unsure whether to visit A&E for their condition, or have an urgent medical need outside of our opening hours, should call NHS 111.
999
Patients with very serious or life-threatening health conditions, such as chest pain, severe bleeding or loss of consciousness, should phone 999 without delay.
