TRAVEL INFORMATION

If you require any vaccinations relating to foreign travel you need to fill in the form below and return it to the surgery. An appointment will then be made to suit your needs with a practice nurse.

It is important to fill in the form as early as possible to allow assessment to take place. You may require an appointment at least 6 weeks before you travel. Many vaccines have to be ordered as they are not a stock vaccine.  Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.

Private prescription for drugs required solely for purpose of travel abroad - £15.00 (+23.50 administration charge)
